Nurse (PACT)-Primary Care in Boise

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care within the Patient Aligned Care Team (PACT). Provides sex-specific direct Veteran care and care coordination, health promotion, education, and coaching on wellness, disease prevention, and chronic care management throughout the continuum of care.
